56TH MEDICAL GROUP
The 56th Medical Group, located near Phoenix, is an outpatient only Medical Treatment Facility (MTF), which serves more than 84,000 beneficiaries (active duty military members, retirees and their families) in the immediate area. The Medical Group is accredited by the Joint Commission of Accreditation of Health Care Organizations. The Medical Group, with more than 600 assigned personnel, has a three-pronged mission that emphasizes providing great healthcare, promoting safety and wellness, and military readiness.

MEDICAL GROUP "HOT SPOT"
56 MDG continues changes

- Are you pregnant, or are you thinking about having a child? Obstetrical services have been fully transferred to civilian network providers (if you are newly pregnant or want more information about network Obstetricains, please see the Referral Management Center - details below). Some women's health services have returned to the Women's Clinic located on the medical group's third floor. To make an appointment, please call (623)856-2273.

- Work on the flight line? A "Back-shop?" Know you need an Occupational examination? All Occupational examinations are now completed by the Aerospace Medicine Squadron, bldg 810.

- Moving overseas or want to know what a "Q-code" is (Q-code is assigned when a dependant must see a clinical specialist at least two times a year - even dental or optometry related)? Dependant OverseasClearances and the Special Needs Program have been transferred to the TRICARE Operations & Patient Administration Flight, located on the second floor of the clinic. - Does your provider want to refer you to the network to see a specialist? Stop by the Referral Management Center (RMC) and let us make that needed appointment for you. The RMC is newly remodeled and located on the second floor fo the clinic. For more information, please call (623) 856-9377.

CARE AND SERVICES
The 56th Medical Group has a commitment to its beneficiaries by providing essential services in a timely manner. The Medical Group's leaders are responsible for ensuring the availability of services to meet the needs of our population. Services will either be provided directly or through referral, consultation or contractual arrangement.

The time frames within which patient care services will be provided are:
*Primary care for acute illness care shall not exceed one day.
*Primary care for routine visits shall not exceed one week.
*Primary care for a well visit shall not exceed four weeks.
*Specialty care wait times shall be determined by the primary care manager based on the nature of the care required, but shall be no longer than four weeks.

Hours of Operation:
The hours of operation for most of the outpatient clinics are: Monday through Friday: 7:30 a.m. to 4:30 p.m.

The hours of operation for the main satellite pharmacy are:
Monday through Wednesday and Friday 7:30 a.m. to 4:30 p.m.
Thursday 8:30 a.m. to 5:30 p.m.

The commissary pharmacy pick up location hours are:
Monday through Wednesday and Friday 9 a.m. to 4:30 p.m.
Thursday 9 a.m. to 5:30 p.m.

The outpatient pharmacy is open Monday through Friday: 7:30 a.m. - 4:30 p.m.

SERVICES AND APPOINTMENTS
The 56th Medical Group DOES NOT have emergency services. If you are seeking emergency care (life, limb, or eyesight) call 911 or please proceed to the nearest emergency room (add as hyperlink w/ map of local ERs).

All Tricare Prime enrollees are required to obtain authorization from their Primary Care Manager before seeking Urgent Care! Please contact 623-856-CARE (2273), including after duty hours.

To make an appointment contact our Central Appointments Line at 623-856-CARE (2273). The Central Appointments Line is open from 6:30 a.m. to 4 p.m., however active duty personnel have priority from 6:30 a.m. to 7 a.m.

To avoid a potential wait time on our Central Appointments Line visit www.tricareonline.com and register each family member to make your appointments via the web 24 hours a day, 7 days a week.

To cancel appointments contact the Central Appointments Line 24 hours a day, 7 days a week.

The time frames within which patient care services will be provided are:

Primary care for routine visits shall not exceed 7 days. (Routine visit examples: sore limbs, allergy symptoms, specialty consult requests)

Primary care for a well visit shall not exceed 28 weeks. (Well visit examples: annual well woman exam, annual adult/child physical)

Specialty care wait times shall be determined by the primary care manager based on the nature of the care required, but shall be no longer than four weeks.

Outpatient Records is located near the southeast entrance of the Hospital between the Dental Clinic and Outpatient Pharmacy.

Outpatient Records is closed on weekends, federal holidays and 56th Fighter Wing directed down days.

PCSing personnel should pick up their medical records 5 days before final out processing.

LABORATORY
The Clinical Laboratory offers a vast variety of diagnostic tests to over 90,000 active duty and TRICARE beneficiaries. The lab is a highly complex working environment where over 17,000 tests for roughly 15,000 patients are performed each month! Our job is to maintain the highest standards while providing the most accurate and cost efficient testing available. We employ cutting edge technology while maintaining the highest degree of customer service and satisfaction!

The Public Health Office is located in Bldg. 1142

Mission
To prevent disease, disability, morbidity, and death through effective use of population-based public health programs. Conducts epidemiological surveillance and analysis of communicable, environmental, and occupational morbidity and mortality to establish and prioritize strategies for prevention and intervention. Provides programs for both individuals and groups to clearly communicate risks and hazards in the workplace/environment and best practices to safeguard health. Key elements of the Public Health Flight include: Epidemiological Services, Occupational Health, Force Health Management, Deployment Medicine / Medical Intelligence, and Food Safety / Facility Sanitation.

The Bioenvironmental Engineering office, as a part of the Aerospace Medicine Squadron, is located in (Bldg 810).

Mission
Bioenvironmental Engineering's (BEE) mission is to magnify Luke AFB's mission effectiveness through fact-based health risk management, resulting in enhanced force protection and productivity. BEE accomplishes this mission by ensuring healthy and safe workplace environments for Luke AFB personnel and their families through evaluation and control of potential chemical, biological, and physical health hazards.

BEE provides industrial hygiene surveillance to industrial work centers at Luke AFB, investigating, documenting, and recommending adequate controls for potential health hazards in the work center. BEE also provides environmental sampling support to base agencies, including monitoring the base drinking water system.

BEE provides radiation protection surveillance and personnel training for workers whose duties involve the use of radioactive materials, microwave, ionizing, radio-frequency and laser radiation. BEE also provides on-site technical expertise during disaster response to protect personnel and the environment.

MDG CONSTRUCTION

RENOVATION PROJECT (AUG 2008 - FEB 2010)
The 56 MDG is currently undergoing renovation in order to provide an enhanced, more efficient work environment and world-class clinic setting for our beneficiaries and staff members. Please ensure you are making appropriate time adjustments to allow for any construction delays. Because of the renovation parking is limited and some clinics have moved (list of changes below). The project dates will be from August 2008 - February 2010.

MEDICAL APPOINTMENT CANCELLATION POLICY
The 56th Medical Group is trying to keep our Airman mission ready and other beneficiaries healthy, so we have implemented a 24-hour cancellation policy for all our medical appointments. By cancelling your appointment at least 24-hours in advance, we are able to provide needed healthcare to other beneficiaries by re-booking the vacated slot. No-show or late cancellations of medical appointments result in unused appointments. This significantly decreases appointment availability for all beneficiaries. The necessity of rescheduling the missed treatment also ties up future appointments, further diminishing the availability of health care for others.

On average a no-show can cost the 56 MDG $237.45. The impact of not showing for a medical appointment is that other beneficiaries may have to be sent to network facilities for their medical care. No one knows our patients better than we do. Please help us take care of you by keeping your medical appointment or cancel it 24 hours in advance so that it can be reused by calling. You can contact 623-856-CARE (2273), including non duty hours and days (i.e. after 1600, Saturday, or Sunday) to cancel your appointments. If you booked your appointment online using www.tricareonline.com you can cancel it online.

CHECK-IN REQUIREMENTS
All patients are required to have a military identification card and current Third Party Collections "Blue" Card (non active duty only). If you do not have a current blue card you can complete the required paperwork (DD Form 2569) at your check-in location to obtain one. The DD Form 2569 is a DoD annual requirement for all non-active duty patients.

Please ensure you arrive promptly for your appointment to allow for any additional check-in requirements such as medication reconciliation, contact information update, new patient packet, etc.

In most cases the first 15 minutes of your appointment time is for your medical technician to complete a medical screening that includes vital signs, medical history, medication reconciliation, etc. This information is required for your health and safety and to properly prepare your primary care manager for your visit.
